Police Clearances – Form 80 Download – Form 1221 Download

If you have a police record, you will need to supply your police clearance with any visa application.

If you are making a Permanent Visa Application

  • Every person 16 and older needs to supply a police clearance.
  • You need to supply a Police Clearance for every country that you have lived in for more than 12 months in the last 10 years.
  • Sponsors for 820/309 and 300 subclass visa’s are also required to supply Police Clearances.

Police Clearances are valid for 12 months.

Overseas Police Clearances are valid indefinitely if you have not returned to the country at all.

Documents not in English need to be translated.

Form 80 and Form 1221 are only required to be completed if requested by the Department.

File Upload

  • Rename Australian Police Clearances to “AFP – Name.pdf” eg “AFP – John Smith.pdf”
  • Rename Overseas Police Clearances to “OS – Name – Country.pdf” eg “OS – John Smith – Thailand.pdf”
  •  For translated files add “- eng” to the file name. eg “OS – John Smith – Thailand – eng.pdf”
  • Upload the original and the translation.
